Quick Launch Checklist

After signing with Levo, we’ll get up to speed as quickly as possible through the following steps:

  1. Kick off meeting: Discuss needs and hypotheses about what leads your employees to stay or leave. This enables us to customize our approach to ensure the best possible outcomes. 


  2. Review survey questions: We will provide template questions that can be customized to individual employer use-cases. Added questions should be simple, with options ranked 1-5 (where 1 is strongly disagree, and 5 is strongly agree).


  3. Decide your starting line: Decide which employees will be the first to receive the surveys.


  4. Finalize pre-kickoff communications: Tell your employees what Levo is, why we are using it, and what to expect (email template).


  5. Review exit survey questions: We will provide template questions that can be customized to individual employer use-cases.


  6. Upload employee list: Send the in-scope employees with the following data fields:

    • First Name

    • Last Name

    • Email

    • Phone Number

    • Job Function (Nurse, Aide, Other Clinical, Admin, Management)

    • Hired Date

    • Supervisor

    • Supervisor phone number

    • Region (or State)

    • Office Location (or Branch)

    • (Optional) Worker Category
